Under the Data Protection (Charges and Information) Regulations 2018, this PRIVACY POLICY as set out by the GENERAL DATA PROTECTION REGULATION (GDPR) sets out the basis, as therapist and supervisor and workshop facilitator, which any personal data I collect from you or that you will provide will be processed. I annually, register with the Information Commissioner’s Office (ICO).
Please read the following carefully to understand how I will treat and regard your personal data in compliance with the GDPR and my insurers Holistic Services Insurance. I am more than happy to discuss any questions you may have. A. HOW I GATHER YOUR PERSONAL INFORMATION
Information that you provide by emailing me via my website www.counsellinginamersham.co.uk, BACP Find a Therapist, SPI website, UCKP, Counselling Directory, IFS directory or by phone or text to request further information about my psychotherapy, supervision or workshop service.

As we are meeting online using the communications software ZOOM I confirm I will not use the facility to record sessions.
After considering the need for using CCTV, I have decided it is necessary for the prevention and detection of crime and for protecting the security of the premises. I do not use the system for any incompatible purposes and conduct regular reviews of the use of CCTV to ensure that it is still necessary and proportionate. The recorded information is retained, safely, for a maximum of 42 days and a minimum of 30 days.
B. WHERE I STORE YOUR PERSONAL DATA
Personal data that I collect from you via any form of contact by email or phone or CCTV will be kept securely and are password protected.
The hand-written assessment form, with your contact details on, is kept securely within a lock-up cabinet and is kept separate from brief content notes which I store on my computer system.
The brief content notes on my computer system are anonymised, using your initials only and are password protected.
All notebooks with tracking notes , including my supervision notebooks, are secured within a lock-up cabinet, are anonymised and are kept separate from the assessment form with your contact details on.
I take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this privacy policy.
C. USES MADE OF THE INFORMATION
I use your contact details to allow me to provide you with information of the service that you request from me; to allow you to tell me about changes in your availability, if you choose to do so, and to notify you about changes to my availability.
I use the brief content notes saved on my computer system to carry out my obligation arising from the agreement entered into between you and me (separate to this policy).
As part of my commitment to providing a professional service, I attend supervision regularly. This is bound by a confidentiality contract (separate to this policy) and furthermore to protect your identity I use your first name only.
D. HOW LONG I KEEP YOUR NOTES FOR
I will retain your contact details and brief content notes on my computer system for as long as we are working together and up to 5 years after we end in accordance with Holistic Services Insurance.
Similarly, as part of my ongoing commitment to your safety, the assessment hard copy with your contact details on will be safely disposed of after 5 years.

Similarly, similarly as part of my commitment to your safety, all my supervision notebooks will be retained for up 5 years after supervision ends after which they will be safely disposed of.
E. YOUR RIGHTS
You are entitled to view, amend, or delete the personal information that I hold. All requests have a month to be carried out.
F. IN THE EVENT OF A DATA BREACH
I have a legal obligation to report a data breach to you and the INFORMATION COMMISSIONERS OFFICE (ICO) within 72 hours.
G. DISCLOSURE OF YOUR PERSONAL INFORMATION
In the event of my incapacity or death your personal contact information will be disclosed to my two clinical executors of my professional Will so that they can notify you.
If I am under a duty to disclose or share your personal data in order to comply with any legal obligation. For example, if I am subpoenaed to court, or as a legal requirement such as safeguarding children or vulnerable adults, terrorism or money laundering.
H. COMPLAINTS
My work is covered by the ‘UKCP Code of Ethics and Professional Practice’ and ‘Complaints and Conduct Process’. Any complaint would be heard in the UK and any disputes under the contract will be governed by the civil law of England and Wales
CHANGES TO OUR PRIVACY POLICY
25. I will notify you of any future changes to this privacy policy.
By signing this Privacy Policy (or emailing me that you have agreed to this policy) you are consenting to my use, and storage of your personal information, you have disclosed to me, as detailed above. You have the right to withdraw your consent at any time.
